CONIC LAB | Informed Consent Form De La Salle University College of Computer Studies You are being asked to participate in a research entitled, DEVELOPMENT OF A GAME-BASED LEARNING ENVIRONMENT FOR SENIOR HIGH SCHOOL PRECALCULUS FOLLOWING AN OUTCOMES-BASED METHODOLOGY You must be 18 years or older to participate in this player-testing activity. If you are under 18 years old, please ensure you have your parent or guardian’s consent to participate in this study. Your participation is voluntary. Please carefully read the information below and do not hesitate to ask any questions regarding the end-user testing that may not be clear to you. The player testing activity is conducted by senior BS Computer Science students, namely Geryco Dionisio, Alfred Oliquino, Joshua Ranjo, and Gwyneth Silverio as part of their requirements for completing their undergraduate thesis. The Conic Lab software is developed under the supervision of Mr. Ryan Samuel Dimaunahan from the College of Computer Studies. A. INTRODUCTION/PURPOSE The purpose of our game, Conic Lab, is to create a game-based learning environment that can help with learning Precalculus, specifically Conic Sections through the players’ different attempts to solve the puzzles in the game. B. PROCEDURE To participate in this end user testing activity, you need to perform the following: 1. Take an online pre-test. 2. Play the GBLE “Conic Lab” for 1 hour or until game completion. a. A researcher will be observing your gameplay at this time via screen-sharing software or in person, depending on the study setup. 3. After 1 hour has elapsed or after the game completion, you will answer a post-test. 4. After completing all the items, you will answer a survey questionnaire and participate in a debriefing or short interview about your experience. The data collected from your answers to the survey forms will be used to determine the effectiveness of the game in evaluating your ability to recognize the equations and characteristics of the different types of conic sections, as well as your ability in solving situational problems which involve conic sections. Note that we will not perform any video or audio recordings while you are using the software and doing the debriefing interview. C. POTENTIAL RISKS AND DISCOMFORT The use of the software will not pose any potential risks or physical harm to you. Your only difficulties may only come from the inability to solve the given problems correctly. Conic Lab also does not require heavy space or graphics which allows you to use it in practically any personal computers. D. POTENTIAL BENEFIT TO SUBJECTS AND/OR TO SOCIETY Your participation in this activity will provide insights to the project team on the potential use of the software by actual high school students who are learning the topic of Conic Sections.
