HISTORY OF PRESENT ILLNESS is a 43-year-old male who was riding his E-bike when he was struck by an automobile on 10/18/2022. The patient states he was flung by the bike over 15 feet and hit the ground. The patient’s bike was totaled. The patient was taken by ambulance to Sutter Medical Center in Sacramento where x-rays were obtained. The patient’s x-ray on 10/18/2022 of his right shoulder shows separation in the AC joint. No fractures were seen. The patient subsequently was treated by primary care physician. The patient still walks with a limp today. The patient has bilateral knee pain from the collision with difficulty with stairs with knee giving way. Pain with sitting for prolonged periods of time and swelling of the knee. The patient denies locking, catching or clicking at this time. The patient states that his right ankle feels unstable as he has burning with pain and numbness in the medial aspect of the ankle. The patient’s elbow has numbness on the medial side as well as numbness that continues to down to the small finger of his right hand. The patient states that he has obvious deformity to his right shoulder at the AC joint consistent with the x-rays with separation of the AC joint. The patient has had an abrasion to the side of his body. The patient has been unable to work since the collision. The patient also states that he has mid-to-low back pain that is persistent. The patient has difficulty with sleeping at night from the chronic discomforts in the shoulder and low back. PAST MEDICAL HISTORY Noncontributory. MEDICATIONS Baclofen and ibuprofen.
