SAMPLE RECEIVING PROCEDURE i. Use standard precautions for collecting and handling all specimens. ii. The entire specimen must be appropriately labelled with patient name, date, IPR number, source, and time of specimen collection. iii. For each specimen received in the department check the name and unique identification number (IPR) on the specimen and requisition slip. iv. Read requisition slip for any special remarks/request for special processing. v. Receiving and inspection should be confirmed by the technologist / technician by signature and date and time on the receiving form. vi. Consult with supervisor/ resident doctor or consultant, in case of any query regarding specimen, or if special test needs to be performed. vii. After collection all specimen should be deliver in the laboratory as soon as possible. Specimen for bacterial culture should be kept at room temperature. viii. If there is a delay in transport, sample should be refrigerated, urine, stool, respiratory specimen should be transported to laboratory on ice. ix. While blood specimen and CSF should be kept at room temperature. Separate the stat, urgent and ER samples. Process them first separately. x. Receive the sample in computer LIS after matching and requisition slip details do the entry in the respective sample entry register. xi. Now mark the department number on IPR slips and sample tubes / container. Do the CRC of each slip on LIS by given slip number on specific request form such as RC, BC and UC etc. xii. Forward sample to the respective bench for further processing. Incubate blood cultures in instrument while manual blood culture bottles (if received) kept it in ambient incubator. xiii. If save sample request received according to retention policy criteria. Assigned bench technologist is responsible for the above mentioned procedure and also responsible for performing, documenting and maintaining relevant records where applicable.
