Purpose This document defines the procedures and steps for establish test reports and correctly reporting the test results 2. Scope This procedure applies to all Conformity Assurance laboratory test reports. 3. Definitions/abbreviation Test Determination according to requirements for a specific intended use or application Note 1 to entry: If the result of a test shows conformity, it can be used for purposes of validation Specification Document stating requirements EXAMPLE: Quality manual, quality plan , technical drawing, procedure document, work instruction 4. Design and develop daily laboratory work sheet and final test report taken into consideration the following common requirements: 6.1.1.1. a title (e.g., Test Report). 6.1.1.2. The name and address of the laboratory” Conformity Assurance Laboratory” 6.1.1.3. the location of performance of the laboratory activities i.e., address 6.1.1.4. unique identification that all its components are recognized as a portion of a complete report and a clear identification of the end. Final report for CONFORMITY ASSURANCE ELECTRICAL LAB In DAMMAM shall be coded as Follow CAED YYXXXXZZ Where YYXXXXZZ is sample code as per mentioned in QP-7.1 6.1.1.5. the name and contact information of the customer. 6.1.1.6. identification of the method used. 6.1.1.7. a description, unambiguous identification, and, when necessary, the condition of the item. 6.1.1.8. The date of receipt of the test or calibration item(s), and the date of sampling, where this is critical to the validity and application of the results. 6.1.1.9. The date(s) of performance of the laboratory activity. 6.1.1.10. the date of issue of the report. 6.1.1.11. Sample provider or if available reference to the sampling plan and sampling method used by sampler where these are relevant to the validity or application of the results. 6.1.1.12. a statement to the effect that the results relate only to the items tested, calibrated or sampled. 6.1.1.13. the results with, where appropriate, the units of measurement. 6.1.1.14. additions to, deviations, or exclusions from the method. 6.1.1.15. identification of the person(s) authorizing the report. 6.1.1.16. clear identification when results are from external providers. 6.1.2. Quality manager shall consult technical and lab manager for ensuring appropriate structure and information for daily report and final test report 6.2. Technical manager shall 6.2.1. ensure the timelines, accuracy and completeness of all testing information reported by the laboratory, and reviews violative worksheets prior to release by the laboratory. 6.2.2. ensure that laboratory reporting data comply with the data and information recording procedures; and review testing findings and supporting documentation generated by the testing responsible 6.3. lab staff shall:

Capture and record accurate and complete testing information on worksheets, and other documents in accordance with the instructions of this procedure; and 6.3.2. To complete worksheets and record testing observations and findings 6.4. Collecting, recording, reporting and analysing test results: 6.4.1. Lab staff and technical manager shall collect, record, report and analysis test results accurately, unambiguously and objectively in daily worksheet/t=raw data / test results report code: 6.4.2. Test results shall include customer requirements /needs as agreed in contract / test request/ tenders 6.4.3. All laboratory results will be stored in the testing worksheet immediately when they are ready. When all results of different tests of the sample have been fed into the testing worksheet the test report will be generated. The report is titled “Test Report” and has constant format 6.5. Storage, retrieval and retention test reports should be stored in ascending order for one year in their file. 6.6. Opinions and interpretation 6.6.1.Opinions and interpretations included in a test report may comprise, but not be limited to the following: 6.6.2.An opinion on the statement of compliance/non-compliance of the results with requirements. 6.6.3.Comparison to national or international standards can be added if required. 6.6.4.fulfillment of contractual requirements. 6.6.5.Recommendations on how to use the results. 6.6.6.Guidance to be used for improvement. 7. Decision rule When the laboratory determines the statement of conformity or non-conformity according to the standard, the following decision rule is applied: • When judging the results, the expanded value of uncertainty is taken into account, which is calculated on the basis of the standard uncertainty multiplied by an increasing factor of 2 (i.e. k = 2) to give a confidence score of 95%, and the measured value alone is not relied upon. • If the measurement result is plus/subtract the measurement uncertainty value with a coverage probability of 95%: 1- It falls within the limits of acceptance and rejection stipulated in the standard specification used to conduct the tests (case 1 in Figure 1), and the final decision is (accepted). 2- It falls outside the limits of acceptance and rejection stipulated in the standard specification used to conduct the tests (case 4 in Figure 1), and the final decision is (rejected).
