Answer sheet IG1_IGC1-0027-ENG-OBE-V1 Jun23 © NEBOSH 2023 page 2 of 10 Task 1: Hierarchy of control Question 1 Controls could provide to minimise risk of slip at kitchen with hierarchy of control measure; 1.Elimination: To eliminate the risk of slip, the faulty ice machine should remove from kitchen, from which water leakage observed, so by remove ice machine the risk completely removes from kitchen. 2.Substitution: o Current tiles are found cracked and slippery near mat next to sink and dishwashers’ area when wet, so to prevent slippery surface slip resistance flooring should install. o Faulty ice machine should replace with new well-conditioned ice machine to avoid leakage of water. 3.Engineering control: o New slip resistance tiles should install over the existing tiles of kitchen which will prevent slips of workers while walking at kitchen. o Ice machine dip pan should install on ice machine, which will collect leaked water and prevent spillage of water on floor. o Slip resistance sheet should provide below floors of kitchen so that I could sock the water spilled on floor, as mats placed next to sink and dishwasher became slipper when walk over it, slip resistance sheet will stay stable at one place. o Additional drainage system below ice machine should provide as it will drain leaked water from ice machine, and prevent spillage on kitchen floor. 4.Admin control: o Regular inspection should conduct as manager predict floor condition good as it made up of hard quarry tiles, so not chances of slip. Regular inspection will let them know actual condition of floor. o Maintenance of machine should provide periodically as the ice machine has malfunction issue as a result water leakage from ice machine, periodic maintenance will ensure malfunction issue and address it on time. o Addition cleaner should appoint at fast food outlet the three temporary cleaner working at part time for cleaning work, and newly appointed worker cleaning floor with wet mop. o SSW should implement for right cleaning procedure at of floor should implement as newly appointed workers cleaning floor with wet mop to dry them which is not a right procedure to dry them, they should use dry mop to clean floor and use right quantity of cleaning products to clean floor. o Adequate supervision should appoint at fast food outlet to ensure workers cleaning floor after if there is leakage and using PPE’s provided to them. As after management assume that workers would have worn disposable grip shoes provided to them, and using card boards over wet patch during leakage of water.

Slippery floor” should display at slippery surface area, where workers could aware about risk of spillage on water exist at place. o Tool box talk on regular time interval should conduct at workplace as one worker place card board on wet surface to sock water, but this caused to slip of worker on floor of kitchen, by developing TBT all cleaner and workers could aware of it. 5.PPE’s (Personal protective equipment’s): o Sufficient number of standards – grip, disposal shoes should provide to workers, as after recent accident identified that no shoes were available for worker while they looking for it. o Permanent well grip slip resistance shoes should provide for each worker as after use once disposal shoes need to dispose, this will short out shoes shortage issue. Task 2: Risk evaluation Question 2 (a) To confirm that the risk level is not acceptable Likelihood of accident: • Kitchen floors being cracked by heavy appliance of kitchen • Mats placed next to sink and dishwasher are slippery to walk-on, when it’s wet. • Newly appointed using wet mop to clean a section kitchen area near by the fryer. • Faulty ice machine has a leakage due to malfunctioning which cause to water leakage at kitchen floor. • When newly appointed cleaner cleaning floor. The floor seems slipper, so other cleaner suggested him to use less cleaning products. By consider above condition, likelihood in risk will consider under Very Likely as per risk matrix, Risk rating will be 3. Severity of injury: • By slips at kitchen one worker face serious head injury by hit over floor of kitchen. The worst possibly could be fractures, permanent disability and fata injury as well. So level of severity consider under Major Injury as per risk matrix and risk rating will be 3.
