Outline how the risk assessment was carried out this should include:  sources of information consulted;  who you spoke to; and  how you identified: - the hazards; - what is already being done; and - any additional controls/actions that may be required. I referred the ILO code of practices https://www.ilo.org/wcmsp5/groups/public/---ed_protect/---protrav/- --safework/documents/normativeinstrument/wcms_107826.pdf before starting the risk assessment. This provides the good source of information to all activities. I visited the Fabrication area, ESP area, FGD (Flue Gas De-sulphurisation) area and nearby control building. After consulting the information source, and I spoke with every employee who worked in these three areas. This provides as much information as possible regarding current workplace dangers and control strategies. Through visual examination and verbal communication, this gathered information from the employees. This aided me in determining whether or not the current control measures are adequate to stop the threats that are now present. Provide the new control measures if not. To find out the specifics of the accident rate, including fatal injuries, major injuries, first aid injuries, lost time, non-lost time, and near misses, I looked through the accident records for the previous 12 months. I have discovered the accurate causes of these occurrences thanks to this knowledge, which also enables me to implement suitable control measures. I referred to my notes during the inspection, in which I had jotted down all the important details on any hazards and risks present in the job and workplace. I was able to propose additional preventive measures for the threats that were observed thanks to this knowledge.
