NCM 113 BY TONS and MADS Formulating Goals & Objectives ● Goals - desired outcomes at the end of interventions ● Objectives ○ short-term changes in community that are observed as health team & community works towards attainment of goals; instructions, defining what should be detected in the community as interventions are being implemented. ○ Specific, measurable, attainable, relevant, time-bound (SMART) objectives provide a solid basis for monitoring and evaluation. ● Sample Goal & Objectives of a Community Health Plan ○ Problem: Risk of Maternal complication leading to maternal mortality in Bagong Silang ○ Goal: To reduce maternal mortality rate from 132/100.000 live births to 80/100.000 live births by the year 2015 ○ Objectives: At the end of the year, the community of Barangay Bagong Silang Will: ■ Demonstrate ability to organize groups to participate in community health process from assessment to evaluation ■ Increase proportion of facility-based births from 10% to 15% ■ Lower proportion of untrained hilot attended births from 20% to 10% ■ Reduce prevalence of nutritionally at risk pregnant women to 20% ■ Reduce prevalence of anemia among pregnant women by 20% Deciding on Community Interventions ❖ The group must analyze reasons for people's health behavior & direct strategies to respond to the underlying causes. ❖ If reason is sociocultural: planning team may opt to concentrate on providing opportunities for skills development of traditional birth attendants or exerting more effort to gain trust & confidence of women & their families. ❖ The group should take into considetration the demographic, psychological, social, cultural & economic characteristic of the target population on one hand & the available health resources on the other hand ➢ Example: preference of home delivery over facility-based delivery ❖ If majority of women would choose to have a home delivery because of cost or lack of access of birthing facilities, strategies should be focused on improving facility-based services IMPLEMENTATION: Implementing the Community Health Interventions ❖ “Action phase”; most exciting phase for most health workers ❖ Able to deal with recognized priority health concern ❖ Nurse's role: To facilitate process rather than directly implement the planned interventions. ❖ Intended to enhance community's capability in dealing with common health conditions/problems ❖ Entails coordination of plan with the community and the other members of the health team. ❖ Requires common understanding of goals, objectives and planned interventions among members of the implementing group. ❖ Collaboration w/ other sectors (LGU may be necessary Partnership And Collaboration ➔ AIM: to get people to work together to address problems or concerns that affect them. ➔ Gives people the opportunity to learn skills in group relationship, interpersonal relations, critical analysis and decision-making process in the context of democratic leadership ➔ IMPORTANCE: ◆ The problem are complicated and too many for the nurse & the people or their organization to handle ◆ They must work with other people or groups to increase probability of accomplishing goals ◆ The nurse must plan to establish & maintain valuable working relationships with people: people's organizations, health organizations, educational institutions, LGUs, financial institutions religious groups, socio-civic organization, sectoral groups.
