Rx CNS RS Chemo 4 4 18. Delirium Tremens (Alcohol withdrawal symptoms) - Inj. Diazepam 10 mg intravenously - Inj Thiamine 100 mg intravenously - I.V. 5% Glucose drip - In case of severe hypoglycemia hypertonic glucose is given (50ml of 50% glucose I.V.) RESPIRATORY SYSTEM 1. Acute mild attack of bronchial asthma with infrequent symptoms - Salbutamol aerosol inhalation using metered dose inhaler with spacer (100 microgram per inhalation) 1-2 puffs as and when required or 6 hourly Note: Patient is advised regular prophylaxis with inhalation (through MDI) of steroid and long acting beta-2 agonist such as (fluticasone 125/250 + Salmeterol 25) 2. Status asthmaticus (Acute severe attack of bronchial asthma) - Moist oxygen through face mask or nasal catheter - Nebulization with Salbutamol respiratory solution 5mg in 2.5 ml of normal saline every 10-15 minutes for 2-3 such doses - Inj. Hydrocortisone hemisuccinate 200 mg I.V. 6 hourly. - Inj. Aminophylline 500 mg diluted in 20 ml of 5% glucose given slowly IV followed by continuous infusion of 0.5 mg/kg/hour for about 6 hours. - Inj sodium bicarbonate 7.5% solution 50ml I.V if severe acidosis is present. Patient is advised regular prophylaxis with inhalation (through MDI) of steroid and long acting beta-2 agonist such as (fluticasone 125/250 + Salmeterol 25) 3. Chronic Persistent Asthma: - Metered Dose Inhaler (Fluticasone 125 mcg + Salmeterol 25 mcg) 1 to 2 puffs twice daily - Disodium cromoglycate 20 mg three times a day using a spinhaler (or Nedocromil sodium inhalation 4mg three times a day) 4. Cough - Dry cough (usually associated with URTI) - Linctus Codeine phosphate (10mg/5ml viscous syrupy base) 5 ml three times a day to be sipped and not to be followed by water for half an hour for 5 days. OR - Syp Dextromethorphan 10mg/5ml 10ml thrice daily may be prescribed. 5. Productive cough with thick mucopurulent expectoration (associated with Lower Respiratory Tract Infection) - Tab. Bromhexine 8 mg three times a day - Tab. Azithromycin 500 mg once a day for 3 days 6. Chronic bronchitis - Metered Dose Inhaler (Fluticasone 125 mcg + Salmeterol 25 mcg) 1 to 2 puffs twice daily - Tab. Aminophylline 100 mg three times a day
